Well, I'd disagree a bit with this one - I love a good effects show, with lots of interesting things going on with the lights, the pyro, the moving platforms on the stage, etc.
It really has no effect at all on the musical qualities of what's being performed - but can add to the entertainment value.
After all, the music generally sounds better on a good home stereo system than in a crowded live venue with acoustics that likely could be charitably described as lacking.
Of course, this may have something to do with my educational background....

at one time things like on stage mutilation and theatrics really interested me, then i had my 14th birthday and didnt really care anymore.

Oh, I'm in no way insisting upon a nipple removal.
In fact, I think I would skip anything involving onstage surgery of any type - I just really don't want to see it.
My point was that theatrics have their place, and can make for a good performance on their own - and if they fit well with the music, so much the better. I kind of got distracted from the onstage surgery concept and was thinking about other types of theatrics. My educational background is technical theatre - with an emphasis on lighting and effects - so that was the sort of thing I was actually talking about.
I will add this, though. If you're going to do a performance that has to stand on musical merits alone, be sure that you've got a very good sound engineer. Bad sound will make a good musician sound like a horrible one.
